A new study suggests that a combination of antidepressants (Citalopram, Fluoxetine, Prozac, and Zoloft) is not superior to a placebo for treating aggression and suicidal thoughts. It's not a one-size-fits-all treatment, and many patients don't respond to their antidepressants.

A recent study published in theJournal of the American Academy of Child and Adolescent Psychiatrylooked at the use of selective serotonin reuptake inhibitors (SSRIs) in children with disruptive behavior problems. In the group, the researchers compared the two SSRIs: fluoxetine (Prozac) and citalopram (Celexa).

"Our findings show that selective serotonin reuptake inhibitors, even when taken alone, do not demonstrate superior efficacy for treating aggressive behavior problems in children," said Dr. Robert C. Schoenfeld, M. D., director of the Child and Adolescent Psychiatry program at Child and Adolescent Psychiatry. "These results should prompt clinicians to consider the risks associated with these selective serotonin reuptake inhibitors in children."

SSRIs are widely used to treat depression and other mood disorders. In most cases, they are considered to be safe and effective at treating disruptive behavior problems. In the latest study, Schoenfeld and colleagues at Children's Hospital of Philadelphia analyzed data from 2,531 children with disruptive behavior problems. The study found that children treated with SSRIs had a higher relapse rate (about 50% more likely to relapse) in those who took the drug and a higher rate of suicidal behavior (about 50% more likely to die) compared with those treated with a placebo.

In the study, Schoenfeld and colleagues also found that children treated with citalopram showed a higher likelihood of recurrence in the first year after the drug was stopped (a relative risk of about 0.45).

In addition, they found that citalopram was not better than a placebo for treatment-resistant aggression, or suicidal behavior. Both groups had a significantly higher rate of recurrence in the first year after the drug was stopped, but the rate in the citalopram group also was higher.

The new study also suggests that citalopram may have fewer side effects than other antidepressants. In a recent paper published in the, Schoenfeld and colleagues also found that citalopram may be less effective for aggressive behavior in children with disruptive behavior problems.

According to the study, the risk of recurrence for citalopram was about 3 times that of placebo for treating aggressive behavior problems. Citalopram, also known as escitalopram, was shown to be more effective than a placebo in treating aggressive behavior in children with disruptive behavior problems.

The researchers are not recommending that parents or caregivers take citalopram or other SSRIs while they are taking citalopram for children. In addition, Schoenfeld and his team are also not recommending that parents or caregivers take citalopram or other SSRIs while they are taking fluoxetine for children.
